Not so many years ago the aim was to make medical records short and to the point. This has changed thanks to computers, health insurance companies, governmental assistance and the aim of physicians to get paid. The result is the elongated, inflated medical record that is heavy from many pages, pointless or at least the point is hard to find and difficult to evaluate. Everyone seems to have discovered that when you don’t know the answer it is best covered by a lengthy report or even if you do have an answer, make it difficult for everyone else to find out what it is.
